In Brief The patient with rapidly progressing, neurologically incomplete cauda equina syndrome remains a surgical emergency, and CES due to acute herniated nucleus pulposus fits this profile. Our best evidence tells us clearly that early surgical decompression will offer the best likelihood of recovery and a good long-term outcome. Nonoperative treatment is appropriate for those patients with stable, mild to moderate symptoms or those with chronic deficits.
