Abstract

Actinic prurigo, a chronic photodermatosis, mainly affects the Mestizos of Latin America. Cutaneous lesions are found in the sun-exposed areas, and involvement of the lips can be the first sign of the disease. Our aim was to report a case of lip involvement in actinic prurigo, highlighting its clinicopathologic features. A 38-year-old man was referred for evaluation of a progressive, painful, pigmented lesion on the lower lip, lasting for 12 months. Clinical examination revealed the presence of diffuse erythematous areas associated with pigmentation in the vermillion border of the lower lip. Clinical diagnosis included exfoliative cheilitis and melanoma, and a biopsy was performed. Histologic features included stratified squamous epithelium with acanthosis, hyperkeratosis, atrophy, pigmentation, and eosinophilic exocytosis. The connective tissue showed an intense inflammatory infiltrate with lymphoid aggregates. The final diagnosis was actinic prurigo, and the patient remains under treatment with topical steroid therapy with partial response.
