Abstract

Another name for weightlifter’s shoulder is distal clavicular osteolysis. It is an overuse phenomenon that manifests following repetitive trauma to the acromioclavicular joint (AC joint) and usually seen in weightlifters. Patients will report insidious pain at the acromioclavicular region radiating to the deltoid or trapezius, exacerbated by weight training. On examination, there is point tenderness to the acromioclavicular joint. Cross-body adduction (scarf test) and Paxinos test will be positive with pain reproduced in the AC joint region. X-rays will show loss of subchondral bone in the distal clavicle. Initial treatment includes avoidance of provocative maneuvers, NSAIDS, and activity modifications. If conservative treatment fails, patients will require surgical resection of the distal clavicle.
