Abstract

We are both extremely grateful to the time and attention given to this project by our liaisons at the American Public Health Association (APHA) Press. Special thanks to David Stockhoff, book production coordinator, for his attention to detail in all aspects of the book. The hard work of Brian Selzer (manager of production), Nina Tristani (director of publications), and David Hartogs (senior marketing manager), as well as the Publications Board of APHA, was also invaluable to this project. We especially thank Dr. Judith Hays for her tireless efforts at improving the manuscript; any errors remaining are those of the authors. Omar Khan: The most important supporters of this work, and all my endeavors, have been my wife Salwa and son Zareef, as well as our extended family and friends. The intellectual foundations of this work were laid at APHA, where Greg and I convened discussions to review the major thematic areas of the book. The support of the Association has thus been invaluable in a number of ways. Many of the authors of this book are long‐time colleagues. This project has thus been a globally dispersed and shared one, and despite our busy lives has allowed us to work more closely together. I would like to dedicate this book to those who live and work in megacities, whether slum dwellers or the public health professionals who strive to improve their lives. I hope that work such as this is a step towards bettering our collective human condition.
