Abstract

A 51-year old woman current smoker with a family history of coronary disease and previous percutaneous intervention (PCI) in the left anterior descending (LAD) presented with stable angina. Left ventricular ejection fraction function was 71% and an “intermediate” ostial lesion of the circumflex artery (40% by Quantitative Coronary Angiography) (Fig. 1A). To assess the hemodynamic severity of this lesion we decided to measure Fractional Flow Reserve (FFR). After advancing the pressure wire (St Jude Medical, Uppsala, Sweden) advanced the distal part of the obtuse marginal branch, a complex stenosis appeared (Fig. 1B).
