Abstract

Not all patients who receive Alteplase need to go to the ICU. They can be safely cared for on a Stroke unit with nurses who have been trained to take care of them and have knowledge of stroke. Due to our lack of ICU beds, many times they would end up holding in emergency room. Our Stroke unit nurses were trained in caring for an Alteplase patient in the first twenty four hours. Education was done on Alteplase, Nicardipine, bedside monitors, blood pressure protocols, angioedema treatment, hemorrhagic transformation, how often to take vital signs. By placing the patient directly on the Stroke unit, physical therapy and therapies were initiated on day one. Improved communication and stroke education was demonstrated with patient and family. Data showed that by placing patient on the stroke unit, length of stay was decreased by one day with a cost savings of $2100/day, no missed vital signs or stroke scales and there were no safety concerns.
