Abstract

Marchiafava - Bignami disease is a progressive alcoholism related neurologic disease characterized by corpus callosum demyelination, necrosis and subsequent atrophy which occasionally occurs in non alcoholics but chronically malnourished. Among the many brain alterations promoted by chronic ethanol consumption, brain shrinkage due to cortical atrophy is the most striking one, it is related to alcoholic dementia and to atrophy of the corpus callosum. Although clearly dependent on age, brain shrinkage is more intense among heavy drinkers. MBD is a rare complication of chronic alcoholism and malnutrition which is most prevalent in men between 40 and 60 years of age. We are presenting here a case of 32year old alcoholic came to us with chief complaints of memory disturbances, decreased selfcare, decreased interaction with family members, provoked anger, sleep disturbances and disoriented to time and place for 2 months, with history of chronic alcohol consumption. A provisional diagnosis of Delirium was made and started him on Tab. Haloperidol 1.5 mg HS, T. Pregabalin 75 mg BD, T. Memantine 5mg OD, Thiamine and folic acid supplements for 7 days which showed no improvement in symptoms, on imaging found to have thinning of Corpus Callosum.
