Abstract

Secondary hyperparathyroidism is a well-established post bariatric surgery complication and is often due to calcium and Vitamin D malabsorption and deficiency. Primary hyperparathyroidism post bariatric surgery is rare and often poses a diagnostic challenge. We describe a case of normocalcemic primary hyperparathyroidism (NPHPT) in a patient with a history of bariatric surgery who was successfully treated with parathyroidectomy. NPHPT is characterized by persistently elevated serum PTH levels in the setting of normal albumin-adjusted and ionized serum calcium levels, and by the absence of secondary causes of PTH elevation.
