Abstract

Reported here is a case of rubella with an abnormally delayed and weak antibody response to the viral haemagglutinin (HA). Initially, neutralizing antibodies and IgM class antibodies were detected with no activity in the haemagglutination inhibition (HAI) test. A serum taken seven weeks later showed seroconversion by HAI, single radial haemolysis and IgG immunoassays. Specific polypeptide and epitope binding assays confirmed the absence of antibody reactivity against the HA epitopes of the E1 glycoprotein in the first serum. This reactivity was detected with the second serum but was markedly weaker than that normally seen after acute rubella infection. Thus, the production of HAI and neutralizing antibodies do not always correlate and this could lead to difficulties in the early diagnosis of rubella.
