•AD calpainopathy-3 affects abdominal wall muscles in addition to proximal limb and paraspinal muscles. •Fatty replacement of internal oblique muscles can be a clue to the diagnosis of calpainopathy-3. •Axial muscle imaging can help define the extension of affected muscles especially in predominant axial myopathy.
