Abstract
Question: A 56-year-old auto mechanic man presented with a 6-week history of fever, chills, drenching night sweats, malaise, nausea, abdominal pain, and distention associated with a 15-kg weight loss. His past medical history was significant for ileocolonic Crohn's disease, diagnosed 20 years ago, asymptomatic on mercaptopurine for the past 10 years. He had remote history of 30 pack-years of smoking, but stopped smoking 20 years ago.
