A 43-year-old woman presented with painful facial swelling after dental surgery. The diagnosis of surgical emphysema could be established by precise history taking and a thorough clinical examination with detailed palpation revealing typical subcutaneous crackling. This case demonstrates a rare, but important, differential diagnosis of facial swelling of which every dermatologist should be aware.
