Abstract

THE INTEGRATED case management in primary and community team (IMPACT) project was established in the Mersey-side area in 2000 to help empower people living with long-term medical conditions, such as coronary heart disease and chronic airways disease. The project's objectives were to improve their quality of life and to reduce GP contacts, hospital admissions and pharmacy costs.
