Abstract

A 33-year-old African American woman experienced two weeks of low-grade fevers, fatigue and decreased appetite. She developed melena which prompted her to present to the emergency room. Physical exam was significant for hepatomegaly. Urine pregnancy test was positive. Laboratories included hemoglobin 8.5 g/dL, total bilirubin 1.2 mg/dL, alanine transaminase (ALT) 29 IU/L, aspartate transaminase (AST) 70 IU/L and alkaline phosphatase 137 IU/L. Bloodwork also revealed a beta human chorionic gonadotropin (hCG) level of 74,772 mIU/mL that increased to 173,087 mIU/mL after 5 days.
