Abstract

A 34-year-old woman, gravida 4, parity 0, with 3 previous abortions, presented to the emergency department at 12 weeks 5 days of pregnancy. She was experiencing a sudden onset of lower abdominal pain with diaphoresis and nausea for one hour. On examination, she was afebrile, with a pulse rate of 104 beats/min, blood pressure of 115/71 mm Hg, hematocrit level of 35.3%, and WBC count of 12,600/μL. Point-of-care ultrasonography demonstrated intraperitoneal free fluid. The gestational sac containing the fetus was seen high in the right side of the fundus, surrounded by a thinned asymmetric myometrial mantle and an empty endometrial cavity (Figure and Video E1 [available online at http://www.annemergmed.com]).
