Abstract
A 79-year-old woman presented with a new onset generalized tonic–clonic seizure. She also complained of a 4-day history of nausea, and headaches. Her vital signs were stable. She had intermittent confusion with an otherwise normal neurological exam. MRI brain (Fig. 1) showed a focal area of hyperintensity with minimal swelling on the medial aspect of the right temporal lobe, no diffusion restriction and lack of contrast enhancement. An electroencephalogram (Fig. 2) showed right hemispheric, periodic epileptiform discharges.
