Abstract

PONTANEOUS hemofiltration has been employed as a “wearable artificial glomerulus” by Neff et al. in 1979’ for the treatment of endstage renal disease. Furthermore, spontaneous hemofiltration has been recently used for acute renal insufficiency and in fluid overloaded patients.’ 3 While useful in acute renal failure, its disadvantages prevented reliable treatment of chronic renal failure. The few attempts to treat uremic patients by continuous hemofiltration failed because of several reasons. Large amounts of fluid had to be replaced, continuous anticoagulation was required, and patients found it an uncomfortable burden. r 4 6 We have tried to overcome some of these drawbacks by designing a new machineless dialyzing apparatus. We report preliminary clinical results of this new therapeutic procedure employed in four uremic patients treated for up to 6 mo.
