The purpose of this study was to explain and present a model of managerial competencies affecting the performance of social security hospitals in Tehran, Iran. Methods: This was a qualitative study. A purposive sampling was applied. The participants were senior managers of social security medical centers and experts in hospital management and administration. Data was collected through in-depth semistructured interviews and continued until saturation. A thematic approach was used to analyze the data. The results led to the identification of 4 comprehensive themes, 10 organizing themes and 40 basic themes. Comprehensive and organizing themes included knowledge and work experience (specialized knowledge, general knowledge, work experience), individual characteristics (moral characteristics, personality traits), interactive competencies (communication skills, internal-organizational communication, external-organizational communication), and executive competencies (managerial competencies, leadership competencies) Conclusion: The use of identified themes can be useful in employing competent managers or be used as the basis of routine inservice training courses for hospital managers in medical centers.

A gap exists in the extant literature on linking information technology governance vis-à-vis hospital performance. Specifically, this work offers insights to fill this gap by adopting the COBIT 5 framework and ISO 38500 standards in examining the relationship between IT governance principles and balanced scorecard (BSC) dimensions of hospital performance. The proposed study investigated the influence of IT governance on Tehran's hospitals' performance. Data were gathered and analyzed from fifty-three (53) questionnaires administered to Health Information Systems (HIS) professionals and IT managers. Results showed that all IT governance dimensions have a positive correlation with hospital performance and IT governance has a significant positive impact on Tehran's hospitals' performance.

Investigating the Effect of Implementing the Lean Six Sigma on Organizational Performance Based on the Mediating Role of Strategic Knowledge Management with Structural Equation Modeling Approach

The main purpose of this study was to investigate the effect of implementing the lean six sigma on organizational performance with the strategic knowledge management as mediating variable in private hospitals in Tehran- Iran. The research method was based on purpose of applied research and in terms of data collection and the analysis method was survey-correlation. The statistical population of this study were 51 private hospitals in Tehran. Questionnaires were used for data collection. Structural equation modeling was used to analyze the data. The results showed that the implementation of the lean six sigma does not have a positive and significant effect on organizational performance; however, it is effective on the strategic knowledge management. On the other hand, strategic knowledge management has a positive and significant effect on organizational performance. Moreover, the implementation of the lean six sigma has a positive effect on the organizational performance through the mediating variable of strategic knowledge management. The research findings can be used to improve the Organizational Performance of Medical centers and private hospitals through the implementation of Lean Six Sigma.

The study of work-family conflict and job satisfaction among nurses' state hospitals in Tehran city.

IntroductionSince nurses work in various wards, job satisfaction evaluation and work-family conflict investigation among them regarding the ward they work in is highly crucial, because on one hand, there are ample requests for changes in wards, shifts, hospitals, and even job abandonment, and on the other hand, family conflicts and clashes have increased among nurses.MethodsThis correlational research was conducted on 280 nurses who worked in special and general wards of state hospitals in Tehran in 2015. In this study, samples were selected randomly among state hospitals subsidiary to Tehran and Shahid Beheshti universities and social security hospitals in Tehran. Data were collected by means of demographic questionnaire, Smith’s job satisfaction questionnaire, and Net Mayer and Mc Marian’s Work-Family conflict questionnaire. In order to analyze the data, SPSS version 21 software was employed and also descriptive statistics methods, correlation coefficient, t-test, regression, and ANOVA were applied.ResultsAccording to the study’s findings, job satisfaction average scores in the supervisor dimension in special and general wards were medium (44.15, 43.868) (p=0.771), job satisfaction average scores in the work dimension in both special and general wards were relatively medium (30.869, 31.520), job satisfaction scores in promotion opportunity aspect in both special and general wards were weak (14.31, 14.187), also work-family conflict average score was 26.07 in special wards and 25.51 in general wards (p=0.519), and work-family conflict average scores in special wards was 15.71 and in general wards was 14.87 (p=0.420), these differences were not significant.ConclusionThe study outcomes reveal that nurses’ job satisfaction is at the medium level. It is noteworthy that the highest percentage of nurses’ job satisfaction in both general wards and special wards are associated with being satisfied with their head nurse, and work-family conflict is equal in both sets of wards (general and special). It is recommended that more studies in the realm of nurses’ job satisfaction and work-family conflict should be done.

The present study aimed to Planning optimal educational needs and apply it for indicating educational needs of nurses managers and (senior manager) and supervisors (middle managers) in Social Security Hospitals in Tehran, its method is Descriptive and the purpose of research is applied research and development. Methodology: Population in this study include 8 matron, 8 educational supervisor, 32 supervisors and 86 clinical head. A sample population is target population. Instruments of study is questionnaire and it is distributed among sample population after its validity confirmed. then 116 questionnaires were returned. Data were analyzed by using descriptive statistics. Results: According to findings of questionnaire, the educational needs of nursing managers in order of preference are: communication skills, leadership and management skills, perceptual skills, insights, skills and attitudes of clinical training in the field of organizational knowledge not confirmed. Conclusion: Based on the results of this study suggest that educational content courses for nurse managers is commensurate with their educational needs General Directorate of Social Security and the pursuit of education and research in Tehran.

Managerial competencies determining organizational strategic orientation in micro and small enterprises

Objective: The research identified which and how the managerial skills perceived by managers of Micro and Small Enterprises (MSEs) impact the strategic orientation. Method: The theoretical model of relationships between managerial competencies of managers and strategic orientation used in MSEs was evaluated with data collected through a questionnaire (Likert scale) and analyzed by structural equation modeling. Results: It was identified that in MSEs, there are specific relationships between specific models of managerial competencies that better support internal or external strategic guidelines, which is different from the researched literature. This indicates that managers' managerial competence models have a simultaneous influence on orientation strategies in internal and external organizations. Theoretical/methodological contributions: The theoretical contribution is in the business context (MPEs), in which the research results reveal the specificity of relationships between the guidelines of strategies and specific models of managerial competencies; the methodological contribution emerges from a model of competencies and strategic orientations (hypotheses), which can be tested in other business environments, provided that the focus is on managerial competences and strategic orientations. Relevance/originality: The study made it possible to identify which and how the impact of managerial competencies is in the internal and external strategic orientation in MSE's. Management contributions: Identifying the relationships between managers' competencies and the strategic orientation of MSE's brings a managerial contribution, as it allows the identification of behavior patterns and necessary competencies on the part of the leader to develop appropriate strategies for the company and the market.

Evaluating the performance of selected military hospitals in Tehran in response to Covid-19 pandemic: A cross-sectional study.

The rapid spread of coronavirus disease 2019 (Covid-19) led the need to admit a large number of infected people to hospitals in a short period of time, turning them into one of the most important responsive organizations. This study aims to evaluate the performance of selected military hospitals because they carried out a military operation in Tehran in response to the recent pandemic. This is a descriptive-analytical study. The statistical population of this study consisted of military hospitals responding to Covid-19 pandemic in Tehran. A checklist to evaluate the performance of hospitals in response to Covid-19 pandemic (six areas, 23 sub-areas and 152 items) was used as a data collection tool in this study. This tool had six domains, including risk management and planning, coordination and communication, infection prevention and control, diagnosis and treatment, education and training, and resource management. The overall performance of selected hospitals was 63%, which indicated a good performance. The domain of coordination and communication obtained the lowest score. The investigated hospitals had good performance because they had a desirable access to resources. Periodic self-assessment and accreditation is recommended to improve the performance of these hospitals.

Application of a Hybrid Method for Performance Evaluation of Teaching Hospitals in Tehran.

Teaching hospitals often face budget limitations and lack of investment. Therefore, the optimal allocation of budget and resources plays an important role in improving the performance and service quality of these hospitals. This research aims to evaluate the performance and determine the efficiency of medical science university hospitals in Tehran. This study identified and categorized 47 effective factors in the performance assessment of hospital units using the Preference Ranking Organization METHod for Enrichment of Evaluation II (PROMETHEE II) method. Moreover, the performance of 40 medical science university hospitals in Tehran was evaluated using an outcome-based model of data envelopment analysis (DEA) with 4 input and 8 output factors and the assumption of scale-dependent efficiency. The hospitals were also ranked according to the Andersen-Petersen (AP) method. PROMETHEE II results identified 12 factors as the most important in hospital performance evaluation. DEA indicated that 16 hospitals had performance scores below 1 and are thus inefficient. The AP method identified Hospital 28, which had an efficiency value of 4.533, as the best hospital. Given the results of this approach and the identification of a considerable number of teaching hospitals as inefficient hospitals, top managers of medical centers must adopt the necessary planning to improve system performance and realize the optimal application of resources.

Investigating the level of functional preparedness of selected Tehran hospitals in the face of biological events: a focus on COVID-19

PurposeThis study aims to evaluate the functional readiness of selected hospitals in Tehran in the face of biological events focusing on the Coronavirus.Design/methodology/approachThe current study is a cross-sectional, descriptive-analytical study, with the research population consisting of four hospitals in Tehran (Ministry of Health, Social Security, Azad and Military University). This study used data collection tools, standardized functional preparedness tools for hospitals using a biological approach and a standardized checklist of biological event preparations for the American Hospital Association. Interviews with the incident and disaster committee director and observation of each hospital’s existing documents, were used to collect data, which was then analyzed using SPSS-16 software.FindingsAccording to the results, the average percentage of total hospital preparedness in biological events is 36.9%. With 53.3%, the selected military hospital has the most preparation, whereas the Ministry of Health has the lowest preparation with 28.3%. Surge capacity management and communication had the most remarkable preparedness rate of 68.75% (adequate preparedness), biological consultants, meeting management and post-disaster recovery had the lowest preparedness rate of 0% (extremely weak preparedness).Practical implicationsThe average functional preparedness of selected hospitals in Tehran was assessed at an insufficient level in this study. Given the recurrence of disease waves, these results are helpful in increasing hospital preparedness for impending events. Improving preparedness in most areas, especially in post-disaster recovery seems necessary.Originality/valueGiven the COVID-19 pandemic, it is important to assess hospitals’ readiness to increase capacity and respond to this scourge. Few studies have been done in this field in the world. This study investigates this issue in the capital of Iran. The finding of this study suggest authorities’ attention to this issue and the creation of severe and prompt solutions and measures and the use of military hospital experiences to improve biological threat preparedness.

Performance Evaluation of Hospitals under Data Uncertainty: An Uncertain Common-Weights Data Envelopment Analysis

In the context of healthcare systems, the performance evaluation of hospitals plays a crucial role in assessing the quality of healthcare systems and facilitating informed decision-making processes. However, the presence of data uncertainty poses significant challenges to accurate performance measurement. This paper presents a novel uncertain common-weights data envelopment analysis (UCWDEA) approach for evaluating the performance of hospitals under uncertain environments. The proposed UCWDEA approach addresses the limitations of traditional data envelopment analysis (DEA) models by incorporating the uncertainty theory (UT) to model the inherent uncertainty in input and output data. Also, by utilizing a common set of weights (CSW) technique, the UCWDEA method provides a more robust and reliable assessment of hospital performance. The main advantages of the proposed UCWDEA approach can be succinctly summarized as follows. Firstly, it allows for the comparison of all hospitals on a consistent basis to calculate a realistic efficiency score, rather than an overly optimistic efficiency score. Secondly, the uncertain common-weights DEA approach exhibits linearity, enhancing its applicability. Thirdly, it possesses the capability to extend its utility under various other prevalent uncertainty distributions. Moreover, it enhances the discriminatory power of results, facilitates the ranking of hospitals in the presence of data uncertainty, and aids in identifying the sensitivity and stability levels of hospitals towards data uncertainty. Notably, in order to showcase the pragmatic application and efficacy of the uncertain common-weights DEA model, a genuine dataset has been utilized to evaluate the efficiency of 20 public hospitals in Tehran, all of which are affiliated with the Iran University of Medical Sciences. The results of the experiment demonstrate the efficacy of the UCWDEA approach in assessing and ranking hospitals amidst uncertain conditions. In summary, the research outcomes can offer policymakers valuable insights regarding hospital performance amidst data uncertainty. Additionally, it can provide practical recommendations on optimizing resource allocation, benchmarking performance, and formulating effective policies to augment the overall efficiency and effectiveness of healthcare services.

Association between Managers’ Competency and Technical Efficiency in General Hospitals in Iran

Introduction: Inefficient management of hospitals leads to the substantial waste of resources. Therefore, hospital efficiency has recently been a major concern among researchers. The present study aimed to evaluate the association of the management competency and technical efficiency in the general hospitals of Tehran, Iran. Materials and Methods: This descriptive-analytical, cross-sectional study was conducted in 28 general hospitals in Tehran, Iran in 2015. In the first step, a survey was performed among 224 managers at different levels in the hospitals using a researcher-made management competency self-assessment questionnaire. In the second step, technical efficiency in the selected hospitals was measured by the Data Envelopment Analysis (DEA). Data analysis was performed using SPSS, Enterprise Management Software (EMS), and Deap1, 2. Results:The average of competency was relatively high among the hospitals managers. However, more than half of the hospital performances were observed to be inefficient. Moreover, the results indicated that the competency of hospital managers was significantly correlated with their field of education, participation in training courses, and managerial experience. Also, a significant, direct association was observed between the competency of managers and technical efficiency in the studied hospitals. Conclusion: According to the results, although the level of technical efficiency was favorable in the studied hospitals, the hospital managers with high competency should further develop and improve their performance and efficiency in these hospitals.

Time management behaviors of head nurses and staff nurses employed in Tehran Social Security Hospitals, Iran in 2011

Background:Effective time management is considered important for managers for achieving the goals in an organization. Head nurses can improve their efficiency and performance with effective use of time. There has always been a lot of disagreement in understanding time management behaviors of head nurses; therefore, the present study was conducted with an aim to compare the understanding of head nurses and staff nurses of the time management behaviors of head nurses employed in Social Security Hospitals in Tehran, Iran in 2011.Materials and Methods:This was a comparative descriptive study in which 85 head nurses were selected through census and 170 staff nurses were also selected through simple random sampling method from hospitals covered by the Social Security. Data collection was done through a standard inventory with high validity and reliability, which consisted of two parts: Socio-demographic characteristics and time management inventory. The obtained data were analyzed by descriptive and inferential statistics through SPSS software version 13.Results:Mean score of time management in head nurses’ viewpoint was 143.22 (±18.66) and in staff nurses’ viewpoint was 136.04 (±21.45). There was a significant correlation between the mean scores of head nurses’ time management and some of their socio-demographic characteristics such as gender, clinical experience, passing a time management course, and book reading (P < 0.05). There was a significant correlation between the mean scores of staff nurses’ time management and their clinical working experience, education, using time management approach, and type of hospital (P < 0.05). The majority of head nurses (52.9%) believed that their time management was in a high level; besides, most of the staff nurses also (40%) believed that time management of their head nurses was high. However, there was a significant difference between the perceptions of both groups on using Mann–Whitney test (P < 0.05).Conclusion:With regard to the importance of time management and its vital role in the quality of nursing care for clients, and also the fact that head nurses believed more in their time management behaviors, they are expected to improve organization's goals and developments in order to modify the weaknesses and shortages and promote the skills and capabilities of their staffs and also resolve the disagreement on the understanding of time management. Moreover, effective time management training programs can be an important step for staff nurses and their head nurses.

The Effect of Preoperative 25-Hydroxy Vitamin D Supplement and Surgical Site Infection

Surgical site infection (SSI) is one of the most important complications of surgery and is known in quality improvement programs as a very important indicator for evaluating the performance of surgeons and hospitals. In recent studies, interesting effects for vitamin D such as antimicrobial effects, wound healing, immune regulation, etc. have been considered and the effectiveness of this vitamin on the above has been proven in laboratory environments and animal models. Therefore, the present study was designed and performed to evaluate the effect of vitamin D tablets before surgery on surgical site infection in patients referred to Shariati Hospital. This study was performed as a randomized controlled trial (RCT) on 200 patients who underwent surgery in Shariati Hospital in Tehran and in the general surgery department between 1397 and 1398. Patients were randomly assigned to two groups of 100 persons, including intervention and control. In all patients, vitamin D levels were measured and recorded 15 days before surgery. The intervention group included patients who received 2 tablets of fifty thousand units of vitamin D seven to ten days before the operation. The second group also included patients who did not undergo any intervention and only in order to maintain blindness of the same size and simultaneously with the first group. They received a placebo. Finally, all patients were followed at intervals of one, three, seven and thirty days after surgery and then compared to the extent of infection at the site of surgery and other desired variables. In this study, 200 persons with a mean age of 47.78 years were examined. 57.5% (115 persons) were female and the rest were male. Overall, 19 (9.5%) of the patients studied underwent surgery at the site of follow-up within one month of follow-up (14 in the control group and 5 in the intervention group). Detection between the two groups was observed in terms of infection (P=0.030). In general, based on the results of the analysis, in the present study, there was a statistically significant relationship between surgical site infection with low serum vitamin D level, increase in the number of hospital days, female gender, wound classification, ASA class of patients and vitamin D consumption before surgery. (P&lt;0.05). The results of this study showed that there is a significant relationship between surgical site infection and consumption of edible vitamin D as well as serum vitamin D levels before surgery so that vitamin D deficiency can be considered as an independent risk factor for infection. Hospitals were considered, including surgical site infections. Therefore, performing preoperative tests as well as performing the required interventions can be very effective in improving this index and reducing surgical site infections.

Leadership effectiveness of hospital managers in Iran: a qualitative study

Purpose The purpose of this paper is to investigate the factors influencing the leadership effectiveness of hospital managers in Iran. Design/methodology/approach Top managers (15), middle managers (10) and operational managers (5) of public, private or social security hospitals in Tehran participated in a qualitative study using semi-structured in-depth interviews. Data were recorded, transcribed and then analyzed via MAXQDA 10 software. Findings The findings were categorized into two main themes of internal and external factors with eight sub-themes. The most important internal factors that had an influence on the leadership of the hospital managers were intra-organizational relations, manager’s personality traits, knowledge, attitude and skills of the manager. The most important external factors included extra-organizational relations, macro-level health policies, access to special financial resources, and social, economic and political factors. Research limitations/implications The results call for a need to understand and analyze the socioeconomic factors influencing managers’ leadership while adopting appropriate strategies. Practical implications The results of the current study can help design training programs for hospital managers, and suggest criteria for appointing hospital managers across the country and this can enhance the effectiveness of their leadership. Health sector policymakers and decision makers should reform the programs that target training and empowerment of hospital managers so that the right people with the right competencies will hold such positions. Social implications The results of this study showed that leadership effectiveness is also influenced by social and external factors. On the other hand, the effectiveness of management leadership can play a significant role in the quality of care provided to the community, patient satisfaction and in hospital social performance through the appropriate management of all hospital resources. Such factors should also be considered in training and appointing hospital managers. Originality/value Although there are several studies on hospital managers’ leadership worldwide, this study is the first to investigate the leadership effectiveness of hospital managers in Iran.

The quality and content of sport management curriculum is an ongoing issue of concern in academia and sport practice. Our study contributes to the endeavour of setting the ideal competence and educational model for sport managers by summarizing the current status quo on how the competencies and the professional standards for sport managers are identified and defined. The recognized competence models for managers as well as a concept of outcome and competency-based education are discussed. The most important current and future competencies for sport managers as identified by 557 practitioners and experts from four sports sectors over the survey carried out from 2017 to 2019 by partners from nine European countries within NASME (New Age of Sport Management) ERASMUS + project, are presented. Using the results of the project, the suitability of a general managerial competency model to determine the competency model for the profession of sports manager was examined. Moreover, the study reflects the extent to which current accreditation standards in the United States and the United Kingdom use competency models.
