Abstract

A physician “report card” in 2010 indicated that one third of patients with diabetes had not received retinal assessment. An inter-sectoral working group investigated determinants of this observation and developed, based on knowledge translation science, a combination of interventions to impact family physicians’ referral practices for retinal assessment. The interventions addressed barriers to recommended best practices through: group learning for family physicians; persuasive communication tools(print and electronic); a referral and consult tool; and approaches to address patient self-management
