Abstract

There is increasing use of programmable electronic systems (PES) in safety-related applications and the task of ensuring that such systems are planned, designed and produced with the appropriate attention to safety is not trivial. In PES, failures due to design mistakes may dominate random hardware failures, so it is crucial to identify hazards early in the design process, then to take appropriate design measures to mitigate the hazards. Hazard analysis is the process of identifying the potential for such undesirable events, and their likely causes and consequences.
