Abstract
Regardless of how effective care delivery has been, good care is often judged on the provision of documentation and record keeping that is timely, clear, full and complete. Documentation of care is the primary source of reference and communication between healthcare workers. This article provides an overview of record keeping and offers guidance to the healthcare assistant and assistant practitioner who are often involved in the maintenance of records in a variety of settings.
