Abstract

Methods.– The aimof the projectwas to clarifywhether provision of a “falls and fracture reduction package” to specific care homes can reduce the incidence of falls. The project ran for 1 year and involved five care homes in our locality. The multidisciplinary package consisted of: – educational events; – review of falls policies/action tools; – introduction of falls champions; – enhanced contact with Community falls team and geriatricians; –medication reviews; – input from Telecare with free equipment; – Tai chi tasters. Results.– Overall there was a 3.5% reduction in falls reported in the care homes involved. More significantly, therewas a 51% reduction in the number of falls related AE –prevented admissions £8368; –number of bed days £12151; –medication reviews £2730.33. The overall cost savings from this project was £18508.33. Key conclusion.– This project highlights that care integration is key going forward in the NHS.
