Title: A discharge education initiative to improve asthma care through innovative teaching material and one-on-one teach-back. Introduction: Discharge from the hospital is a vital point in care when a practice team can change the trajectory of chronic disease. Multiple studies have looked at implementation of asthma action plans (AAP) to alter markers of asthma control with varied but often unimpressive results.
