Abstract
Objective: Milled patient-specific custom plates allow for unparalleled accuracy in complex oromandibular free flap reconstruction. One of the advantages of custom plates is the utilization of strategic predictive holes to secure the cutting guides intraoperatively. These same holes are also designed to line up with holes on the custom plate and therefore considerably simplify positioning of the reconstruction bar in the preplanned position. The cost associated with this technology can be prohibitive. We present an alternative novel technique whereby we manually create predictive holes in the laboratory and combine them with the use of a stock plate. This allows for expedited intraoperative placement without the added expense of a patient specific bar.
