Abstract
Nonsteroidal anti-inflammatory drugs (NSAIDs) are the most important tool to control pain in rheumatic diseases; however, their application is limited by the risk of serious complications in the cardiovascular system and gastrointestinal tract (GIT). The advent of a Russian new drug that is a naproxen and esomeprazole combination (Vimovo™) extends the possibilities of NSAIDS use. This review considers the benefits of both NEC components. The former is naproxen, a traditional NSAIDS that is in common use as an effective analgesic worldwide. Its chief merit is the least cardiovascular risk among all NSAIDs (aside from aspirin). Esomeprazole is a representative of the group of proton pump inhibitors, a potent antisecretory drug that has passed major tests as an agent for the prevention of NSAID-related GIT complications. This drug combination allows patient incompliance to gastroprotective therapy to be eliminated. Large-scale clinical trials have confirmed a considerable reduction in the frequency of GIT complications with NSAIDS use as compared to the standard enteric-coated naproxen, including in patients receiving low-dose aspirin. Comparison of NEC with celecoxib has indicated that the new medication is as effective as a selective COX-2 inhibitor in both efficacy and GIT safety.

Сегодня мы наблюдаем возвращение интереса к «традиционным» НПВП. Логика практикующих врачей очевидна – лучше иметь дело с надежными лекарствами, которые хорошо зарекомендовали себя на протяжении десятилетий, чем использовать новые препараты, с которыми связана опасность развития сердечно-сосудистых осложнений, ведь больных, имеющих кардиоваскулярные факторы риска, не меньше, чем лиц с факторами риска патологии ЖКТ [20, 21]. В ряде серьезных исследований было показано, что опасность кардиоваскулярных катастроф у больных, получающих диклофенак, оказывается даже выше в сравнении с другими НПВП, включая селективные ЦОГ2-ингибиторы. Касающихся кардиоваскулярного риска различных НПВП, оцененного в ходе 17 исследований случай-контроль (всего 86 193 больных, перенесших инфаркт миокарда, и 527 236 лиц контрольной группы) и 6 когортных работ (сНПВП получали 75 520 человек, нНПВП – 375 619, а 594 720 лиц составляли контрольные группы). В итоге риск развития инфаркта миокарда (относительный риск – ОР) для пациентов, принимавших диклофенак, был равен 1,4, в то время как для напроксена – 0,97, для пироксикама и целекоксиба – 1,06, для ибупрофена – 1,07, для индометацина – 1,3 [29] На сегодняшний день диклофенак уже не соответствует современным представлениям о безопасной анальгетической терапии и не может рассматриваться в качестве наилучшего средства выбора среди традиционных НПВП прежде всего из-за риска кардиоваскулярных осложнений
