Abstract

Abstract Congenital piriform sinus fistulas are uncommon and controversial, both in their classification and management. Bilateral cases are extremely rare, and no management paradigm exists. We present a case of an infected left-sided intrathyroidal branchial cleft cyst with concomitant asymptomatic right-sided piriform fistula, successfully treated with surgical excision of left-sided cyst including thyroid lobectomy and right-sided chemical cauterization with 18 months follow-up.
