Abstract

We experienced an extremely rare case of splenic cyst with a high serum CA19-9 level. A 21-year-old woman experienced left upper abdominal pain with sudden onset, and she was admitted to the department with a suspicion of intraabdominal mass. Ultrasonography, computed tomography, and magnetic resonance imaging of the abdomen revealed a large splenic cyst. Preoperative blood examination showed high levels of serum CA19-9 (16, 706U/ml)and CEA (34.5ng/ml). Under a diagnosis of CA19-9 producing splenic cyst, a laparoscopic splenectomy was performed. The cyst was 12cm in diameter and histopathologically epidemoid cyst. Intracystic CA19-9 and CEA levels were quite high (102, 820U/ml and 1, 169.3ng/ml, respectively). On immunohistochemical stainings with anti-CA19-9 and anti-CEA, the epithelium of the cyst revealed positive reactions. However, no malignancy was confirmed. Postoperative course was uneventful. After the surgery, both elevated serum CA19-9 and CEA levels normalized.
