Abstract

Spigelian hernia is extremely rare, and only 13 cases have been reported in the Japanese literature. This report describes a case of Spigelian hernia, with a review of the literature. An 81-year-old woman was seen at the hospital because of a localized bulge and pain in the right side of the umbilical region. The abdominal findings showed a 4.0 × 4.0 cm tender mass in the right side of the umbilical region. By abdominal computed tomography, the aponeurosis under the bulge was found to be defective, and intra-abdominal contents which appeared to be from the mesentery of the small intestine, protruded into the abdominal wall through the defect. The preoperative diagnosis was a Spigelian hernia. At surgery, a hernial sac was found under the thin external abdominis oblique aponeurosis. The hernial sac covered with preperitoneal fat was 5.0 × 5.0 cm in diameter, and the hernial orifice was 5.0 × 5.0 cm in diameter. The hernial sac was ligated and divided at the level of the hernial neck, and the hernial orifice was closed. The patient has been in good health without any signs of recurrence for a year and 9 months after the operation.
