Abstract

Primary perineal hernias are extremely rare type of pelvic hernias found in surgical practice. The chief complaint of almost cases is “a mass in the buttock”. This report describes a case of primary posterior perineal henna with a rare clinical presentation as recurrent paroxymal inguinodynia. A 67-year-old female was admitted to the hospital because of paroxymal right inguinoynia. There were previous histories of chronic gastritis and status vertiginosus. She was delivered of a child normally. The correct diagnosis could not be made from various examinations including ultrasonography, perineal CT, and MRI. Two years and seven months later, exploratory laparotomy was carried out. It was found that a herniation with 1.5cm in diameter existed at the right side of the bladder and the tail side of broad ligament of the uterus, where there was nothing. It was diagnosed as posterior perineal hernia by the palpation from between pervic floor and buttock. Operative procedures included primary closure and repair with broad ligament of the uterus. There has been no recurrence, as of 1 year and 4 months after the surgery.
