Abstract
A case of multiple splanchnic artery aneurysms, which were found out after surgical treatment for bleeding from a ruptured dissecting superior mesenteric artery aneurysm, is reported. A 51-year-old man was suddently seized with an increasing epigastric pain, and was admitted to the hospital. Laboratory data on admission disclosed a slight anemia. In a short time, abdominal distension appeared which was followed by a hemodynamic shock state. Emergency angiography showed massive hemorrhage from a branch of the middle colic artery. The patient was operated on. The bleeding artery was resected for about 3cm successfully. Histological examination showed a characteristic of a dissecting aneurysm. Next detailed angiography revealed multiple splanchnic aneurysms. The locations of these aneurysms were the followings: the proximal portion of the left hepatic artery, the trunk of the superior mesenteric artery and the marginal artery of the colon from the ascending to he hepatic flexure. The aneurysm located at the trunk of the superior mesenteric artery was thought to be difficult to operate, because it contained many intestinal branches. Multiplicity of the aneurysms made this case more complicated. For the present, our choice is a careful observation, instead of further surgery.
