Abstract
背景．原発性肺癌は多臓器に転移しやすい癌であり転移巣は多岐にわたる．その中で皮膚指尖部への転移は非常に稀である．症例．72歳男性．胸部CTで右S10に腫瘤影を指摘された．気管支内視鏡検査を施行し，肺扁平上皮癌と診断した（cT2N2M0 Stage IIIA）．初回化学療法としてcarboplatin+paclitaxelを4コース施行後，放射線療法を開始したが，治療経過中右第4，5指尖部に疼痛を伴う腫脹と発赤が出現し，同部位の穿刺細胞診を施行し，肺癌の転移と診断した．手指転移に対し放射線治療（30 Gy/10 Fr）を施行したが，除痛効果は認めたものの，病変部の壊死，自壊は進行し，他の指尖部にも同様の病変が出現した．2次化学療法としてdocetaxelを施行したが効果なく，その後原発巣の増大，多発皮下転移などが出現し，手指転移から3ヶ月で病死した．結語．肺癌の転移巣として稀な皮膚指尖部転移の症例を経験した．
