Abstract
68歳男性。前医にて肺大細胞癌，転移性脳腫瘍と診断された。黒色便を主訴に当院を受診した。上部消化管出血を疑い，上部消化管内視鏡検査を施行したところ，胃角部に出血を伴う隆起性病変を認めた。前医での上部消化管内視鏡検査では0-Ⅱc様病変であったが，短期間に3cm大の1型胃癌様に形態変化をきたしていた。出血コントロールが困難であったため，腹腔鏡下胃部分切除術を施行した。術後病理組織診断は大細胞癌であり，肺大細胞癌の胃転移と診断した。
