Abstract

This paper describes a case of congenital mesenteric hiatus hernia associated with small bowel obstruction, as well as a review of 144 cumulative cases reported in the Japanese literature.The case was a 54-year-old male with the chief complains of abdominal pain and vomiting. The patient was diagnosed as ileus and underwent laparotomy. During surgery, a 6×5cm defected opening with smooth margin locating at the mesenterium about 10 cm oral-side from the end of ileum was found. It was a hiatus hernia that approximately 60 cm of the ileum protruded through the opening. Moreover, obstruction involving the ileum was found which coincided with the location of the defected opening. Hence closure of the defected opening and colectomy at the obstructed site were carried out. Histological findings of the obstruction included a segmental complete defection of the specific muscle layer, which bore a striking resemblance to that of the atretic site of congenital atresia of small intestine.
