Abstract

A case of recurrent bleed from an ileal Dieulafoy's lesion in a patient with Crohn's disease status post multiple bowel resections is reported. After undergoing two prior ileocolonic resections for life threatening lower gastrointestinal hemorrhages, this patient presented with three episodes of transfusion requiring hematochezia. Two initial colonoscopies were non-diagnostic. The third colonoscopy revealed an actively bleeding vessel in a disease free region of the ileum, consistent with Dieulafoy's lesion. The lesion was treated endoscopically with epinephrine injection and cautery with good hemostasis. This is the first reported case of ilea dieulafoy's lesion in a patient with Crohn's disease. Though life threatening hemorrhage is a known complication of Crohn's diease, we should not omit other causes of lower gastrointestinal bleed.
