Abstract
ビニール加工業に従事する54歳の男性が,職場と関連した咳嗽,発熱を認め当科入院し,胸部X-P上小粒状影, transbronchial lung biopsy (TBLB)で胞隔炎, bronchoalveolar lavage (BAL)でリンパ球分画の上昇を認め,環境誘発試験陽性のため過敏性肺炎と診断した.職場で使用した接着剤にイソシアネートが含まれ,血清, BAL中のイソシアネートに対する抗体及び末梢血リンパ球幼若化反応陽性であり,イソシアネートによる過敏性肺炎と診断した.
