Abstract

Although esophageal diverticulum is not uncommon, it is asymptomatic in most cases and is rarely considered an indication for surgery. Esophageal diverticulum wit a size larger than 5×5cm on radiograms is called giant esophageal diverticulum which is seldom encountered. Here a case of giant esophageal diverticulum affecting the inferior esophagus is described. A 60-year-old man was seen at the hospital because of dysphagia and vomiting. Esophageal fluoroscopy revealed an esophageal diverticulum with the size of an egg. At that time, he was left untreated because the symptoms were mild. Three years later, he visited the department because of aggravation of the symptoms. Gastrointestinal radiography revealed an esophageal diverticulum (base width 3.5cm, size 7.0×5.0cm) in the left wall of the inferior thoracic segment of the esophagus. The patient underwent diverticulectomy. The removed tissue was histopathologically classified as pseudodiverticulum with a slight formation of the proper muscle layer. It appeared to be pulsion diverticulum. In addition to presentation of this case, 20 cases of giant esophageal diverticulum reported in Japan were analyzed with reference to the literature.
