Abstract
A case of Crohn's disease with an enterocutaneous fistula after appendectomy is reported. A 14-year-old girl suffering from abdominal pain for 1 month underwent appendectomy under a diagnosis of acute appendicitis. Five months after the appendectomy, an enterocutaneous fistula connecting to the cecum from the surgical wound developed. Clinical course and radiographic examination suggested Crohn's disease and ileocecal resection was performed. Pathological examination of the resected specimen conformed to be Crohn's disease. Since inflammation of the ileocecum could not be found at appendectomy, Crohn's disease primary affected the appendix would extend to the ileocecum.Recently, patients with Crohn's disease have been increasingly reported in this country. Probable occurrence of Crohn's disease of the appendix should be considered when the clinical course of appendicitis is atypical.
