Abstract

A case of bilateral obturator hernia, which could be diagnosed from the prolapse of the ileus tube into the left obturator foramen, is reported. A 75-year-old female was admitted to the hospital because of vomiting, abdominal pain, and left inner femoral pain lating for 12 days. The abdomen had distended by the time of hospitalization, and she was in a sub-ileus state. On the 6th hospital day, a computed tomography showed the ileus tube prolapsing into the left obturator foramen. As a diagnosis of incarceration of left obturator hernia was made, an emergency operation was performed. At laparotomy, a part of the oral ileum 50cm from the ileocecal valve was incarcerated. The ileus tube had prolapsed into the left obturator foramen by breaking through the necrotic portion of the incarcerated ileum. The great omentum was also found to have herniated into the right obturator foramen. Bilateral orifices of the obturator foramen were crosed with peritoneum, and her postoperative course was uneventful. This is a rare case of the left obturator hernia being confirmed by preoperative CT due to a prolapsing of the ileus tube into the left obturator foramen.
