Paragangliomas are rare tumors affecting 1 to 2/100,000 adults per year. Extra-adrenal paragangliomas presenting as pancreatic/peripancreatic masses are even rarer and pose a diagnostic challenge on fine needle aspiration (FNA), with a high rate of diagnostic error (50% in case reports).
