Abstract
The World Health Organization defines the social determinants of health (SDH) as the “conditions whereby people are born, grow, live, work, and age.”1 Understanding root causes of poor health, such as income and housing, are imperative to improve patient care and public health because the SDH are known to influence health inequities. In addition, Healthy People 2020 has proposed five domains for the organization of SDH. These include Economic Stability, Education, Health and Health Care, Neighborhood and Built Environment, and Social and Community Context (see the Figure).
