A 10–year experience of extraperitoneal laparoscopic radical prostatectomy for localized prostate cancer from a single centre

Abstract

Objective To report a 10–year single–centre experience in extraperitoneal laparoscopic radical prostatectomy (EPLRP) for localized prostate cancer and report perioperative, functional and oncologic outcome. Methods From 2005 to 2015, 418 men with localized prostate cancer underwent EPLRP at urology department of Shanghai Changzheng Hospital. The mean preoperative prostate–specific antigen (PSA) was 15.9±7.0 μg/L and mean age was 67.8±6.7 years. Stage distributions were cT1a–b in 8 cases, cT1c in 198 cases, cT2 in 123 cases and cT3 in 89 cases. Disease recurrence was defined as PSA≥0.2 μg/L in 2 consecutive measurements. Results Mean operation time, mean blood loss and drainage removal time were 144.4±58.4 min, 164.0±231.1 ml and 6.9±4.8 d. There were 4 conversions to open surgery. The urine leak rate was 4.5%, and rate of positive surgical margins was 14.6%. The continence rate was 87.8% (367/418) at 12 month after operation, and the overall PSA recurrence rate at 2 years was 17.9% (75/418). Conclusion EPLRP could be an effective minimally invasive approach for localized prostate cancer, and patients can expect satisfactory outcome in experienced hand.
