Abstract

Objectives. Tc-99m MIBI is a useful method for identifying breast, lung and thyroid cancers. However, only a few reports have focused on the uptake of Tc-99m MIBI in hepatocellular carcinoma. The aim of this study was to investigate the relationship between Tc-99m MIBI accumulation in HCC and the expression of p-glycoprotein-mdr. Methods. We selected 22 patients with hepatocellular carcinoma. Tc-99m MIBI SPECT was performed 10 minutes after intravenous injection of 20mGI Tc-99m MIBI. All patients underwent either liver biopsy or surgery with in 1 week of MIBI imaging. Immunohistochemical study of the biopsy specimens and the resected hepatocellular carcinoma specimens was performed with anti-human p-glycoprotein antibody. Results. There was no uptake of Tc-99m MIBI by tumor lesions in 20 out of the 22 (90.9%) patients. p-Glycoprotein expression was observed in 13 out of 20 (65%) patients with no Tc-99m MIBI uptake. There was no expression in the 2 patients with positive Tc-99m MIBI uptake. Conclusions. Tc-99m MIBI SPECT is a useful non-invasive method for predicting the expression of the p-glycoprotein multidrug-resistance gene in hepatocellular carcinoma patients.
