Abstract
症例は49歳，女性．原発性肺癌にて右肺上葉切除（高分化型腺癌，腺房型，pT1N0M0）を行い経過観察中，3年6ヵ月後のCTにて左下葉に小結節が出現した．9年後には増大し両側下葉に新たに4個の結節が出現した．他に転移病巣を認めなかったため切除の方針とし，胸腔鏡補助下に右下葉1個，左下葉4個の結節の部分切除を行った．組織学的に5病変とも再発転移病巣と診断された．術後療法は行わず経過観察中で3年3ヵ月無再発生存中である．本症例は対側肺葉を含む多発転移で極めて予後不良な群と考えられるが，切除のみで3年間以上無再発で経過しており稀な症例だと思われる．初回手術から再発の期間が長いこと，増殖速度が遅いことが切除適応の因子となるかもしれない．ALK遺伝子変異があり，生検で診断しALK阻害剤を投与する方針も考えられたが，呼吸機能の損失も少なく，化学療法をせずに過ごせたことを考えると手術は有益だったと考える．
