Abstract
First described in the early 1980s, the radial forearm free flap (RFFF) has become a popular choice among surgeons reconstructing the head and neck owing to its versatility, suitability for two-team operating, and reliable vascular anatomy. Following radial artery harvest, perfusion of the hand relies predominantly on the ulnar artery. We present a case of atypical anatomy of the ulnar artery while raising the RFFF and explore its relevance to reconstructive surgery.
