Abstract
閉鎖孔ヘルニアは高齢者に好発し，診断時に全身状態が悪化していることも多い．術式は従来開腹法が一般的であったが，低侵襲手術として鼠径法の報告例が増加している．当院においても鼠径法で手術を施行した7症例を経験した．麻酔は硬膜外麻酔，局所麻酔にプロポフォール，笑気などで軽度の鎮静を加えた．手術は鼠径アプローチにて腹膜前腔に入り，ヘルニアを還納，ヘルニア嚢を処理した後腹膜前腔を十分に洗浄しポリプロピレンメッシュを用いてヘルニア門を広範に覆った．1例に腸管壊死を認め，切除吻合を施行した．術後は特に合併症を認めず，術後歩行開始日の中央値は2日，術後排ガス日の中央値は1日であり，術後在院期間の中央値は10日であった．以上より，鼠径法は低侵襲で回復が早く，全身状態が不良の症例も安全に手術を施行することが可能であり，閉鎖孔ヘルニアに対する低侵襲手術として今後標準術式になりうると考えられた．
