Abstract

Guidelines from numerous societies state that urinalysis for suspected Urinary Tract Infection (UTI) should only be ordered on older adults (age > 65) in the presence of signs and symptoms of a UTI. This is due to a high rate of asymptomatic bacteriuria (ASB) in this population, a condition in which urinalysis may appear positive and urine culture may grow a urinary pathogen, but there is no benefit to treatment with antibiotics. However, urinalysis is commonly performed on older adults presenting to the emergency department (ED) for vague symptoms such as generalized weakness or altered mental status, despite these not being signs or symptoms of UTI.
