BACKGROUND CONTEXT: Excessive pedicle screw compression required for adequate kyphotic deformity correction may lead to intra-operative screw displacement/loosening. This is especially problematic in adult, osteoporotic patients. Currently, surgeons place the rod cephalad and start compressing from “top to bottom” against the cephalad aspect of the pedicle, or vice versa. However, it is unknown which of these two techniques is optimal.
