However, the literature regarding the frequency of regulatory T cells (Treg) in renal cell carcinoma (RCC) compared with normal control is controversial. Also, there is increasing evidence for age-dependent development of Treg cells. We investigated the frequency of Tregs in the peripheral blood of RCC patients compared with age-matched healthy control and compared the prevalence of Treg cells in the tumor infiltrating lymphocytes (TIL) with lymphocyte in normal kidney tissue. METHODS The frequency of Treg cells in the peripheral blood of 62 patients with clinically localized RCC and from 65 healthy controls was evaluated by flow cytometry using specific monoclonal antibodies recognizing CD4(+), CD25 (+) and Foxp3 (+) markers. The prevalence of Treg cells in the TIL and lymphocyte in normal kidney tissue from 54 patients with RCC were also evaluated. RESULTS The median age of the RCC and control group was 55 (<40 years: n=8, 40-49: n=13, 50-59: n=16, ≥60: n=25) and 53 (<40 years: n=9, 40-49: n=18, 50-59: n=16, ≥60: n=22) years, respectively. The median tumor size was 6.0cm (1.7-13.7cm) and 49 patients (79%) had clear cell histology on pathologic examination. There was no difference in the frequency of Treg cells in the peripheral blood in entire RCC patients compared with healthy donors (2.8 ± 4.0% vs 1.9 ± 2.6%, p=0.150). Treg cells frequency was found to significantly higher in RCC patients with large tumor size (≥7cm, 4.2 ± 5.5%) than those from normal healthy control or small to medium sized RCC patients (<7cm, 2.0 ± 2.6%, p=0.016). Furhman nuclear grade did not affect the frequency of Treg cells among RCC patients (Gr 1-2: 2.5 ± 3.6% vs Gr 3-4: 3.0 ± 4.4%, p=0.675). There was no statistically significant difference in the prevalence of Treg cells between TIL and lymphocyte in normal kidney tissue (5.5 ± 9.5% vs 4.8 ± 5.8%, p=0.629).
