Abstract
To improve process plant safety requires preplanning for how plant personnel will respond to an emergency situation such as:•A failed fired heater tube that requires use of snuffing steam to prevent an explosion in the heater firebox.•A centrifugal compressor that is caught in a surging mode of operation. The surges are damaging the compressor's axial thrust bearings and increasing the axial movement of the rotor.•A boiler feed de-aerator that is caught up in a positive feedback loop that will cause hot water to eventually blow out of the top vent.
