Abstract
胃癌に対し胃全摘，結腸後Roux-en Y再建を施行後，続発性小腸軸捻を発症した3例を経験したので報告する．症例1は35歳，男性．2年前に手術施行．突然の背部痛を主訴に受診した．腹部CTにてwhirl signと上腸間膜静脈の途絶像を認め，緊急手術を施行した．全小腸が上腸間膜動脈を軸として時計方向に360°回転していた．症例2は72歳，男性．2年前に手術施行．突然の腹痛・嘔吐を主訴に受診した．腹部CTにて全小腸の拡張とwhirl signを認め，イレウス管による保存的治療にて軽快せず，開腹術を施行した．全小腸が上腸間膜動脈を軸として時計方向に540°回転していた．症例3は66歳，男性．1年前に手術施行．突然の腹痛を主訴に受診した．腹部CTにて絞扼性イレウスと診断し，緊急手術を施行した．拡張した小腸が上腸間膜動脈を軸に時計方向に180°回転していた．3症例ともに捻転解除のみで軽快した．胃切除後の小腸軸捻の報告例について，文献的考察を加えて報告する．
